Clinical Feature Article: Are You Hip to Extended Postop Rehab for Older Patients?

Traditionally, rehabilitation after surgery to repair a hip fracture lasts 8 to 12 weeks. But recent studies have shown that moderate- to high-intensity resistance training after discharge from a standard postoperative program appears to be safe and may even play a role in increasing older patients' survival. Why Force the Issue?    Encouraging forced use or constraint-induced movement therapy in acute and sub-acute care settings isn't always realistic. Based on pilot work by Hammer and Birgitta, a 2-week rehabilitation program performed in a sub-acute setting with restraint of the patient's nonparetic arm generated no greater mobility gains than rehabilitation done without restraint of the nonparetic arm.

First Breast Cancer, Now a Swollen, Painful Arm    Overweight patients are predisposed to lymphedema after treatment for breast cancer but are often cautioned against exercising. Jönsson and Johansson have identified that women with arm lymphedema can participate in a controlled, short-duration, pole-walking program using a compression sleeve without the exercise increasing volume in the edematous arm.

Tubing with Pull Improves Balance    Han, Ricard, and Fellingham found that four simple elastic tubing exercises with the tubing around the unaffected ankle can improve balance in patients with and without lateral ankle sprains. More important, the patients retain the improvement over 4 weeks. Cost-effective and performed at home, elastic tube exercises allow perturbation that requires dynamic adjustments to improve balance.
